 New Client Intake Form is Required for Treatment

Please fill out the Intake form prior to you scheduled session. If you are unable to fill it out prior, you may do so in the office.  Please arrive 15 minutes early in order to do so.

A credit card number is required to hold the        appointment time.

When scheduling your appointment online or by phone, a credit card is needed to book and confirm your session.  The card will NOT be charged until the time of the service, unless you opt to prepay.  Alternative methods of payment (cash, check or gift card) may also be used.

 Cancellation Policy

24 hour notice is required.  If notice is not given within 24 hours, a $50 fee will be charged to your card.  If you do not call to cancel your appointment and are a "no show", you will be charged the full amount of the session.  If a Gift Certificate/Card is the form of payment, monies will be deducted from the total remaining on the card.
